RCA, Problem Solving and Corrective Action

Wellflex has developed a unique 8 step Corrective Action/Preventive Action (CAPA) Process that combines requirements of ISO, API, PDCA, DMAIC and 8D problem solving techniques.


While the CAPA process outlines the approach, the process of root cause analysis and evaluating the appropriate actions needed often involves multiple techniques.  This program includes the use of multiple problem-solving approaches and training on how to apply the tools most effectively:

  • Process Mapping using SIPOC Approach and Turtle Diagram
  • 5-Why Problem Solving
  • Cause & Effect Analysis using Fishbone Diagram
  • SWOT Analysis
  • Prioritizing Risk using modified Facilitated Risk Analysis Process (FRAP)

The most important objective in problem solving is that the actions are effective in achieving the desired outcome.  Our training includes methods to analyze the effectiveness and how to determine the key performance indicators needed.  This includes an in-depth look at multiple tools used for analyzing process control and verifying effectiveness:

  • Pareto Analysis
  • Regression
  • Run Charts
  • SIPOC Analysis
  • Effective Auditing


We offer a unique, in-depth approach to managing continuous improvement projects.  We also offer training on effectively empowering your employees to facilitate improvement initiatives effectively.  This program was developed based on the Wellflex Team experience in multiple industries and has allowed for a cross-functional understanding of how to drive effective “breakthrough” change in any environment.


Process Control

Wellflex offers additional services to manage projects driving improvements on targeted areas of process control.  While this is similar to the program above, this approach may require more advanced techniques to reach desired outcomes.  After initial assessment, the Wellflex Team can facilitate projects including:
